CHPS Ophthalmology Core

Ophthalmology Research Services have moved to the Center for Outcomes Research in Surgery (CORES). Contact Agnieshka Baumritter, Ophthalmology Team Manager, for a start-up call/meeting prior to scheduling research subjects to use ophthalmology services.

How to Request Ophthalmology Services

1. Study Staff must complete the REDCap intake form: Ophthalmology Research Services.
2. Study Staff requesting Ophthalmology Services must ensure the study's OnCore budget reflects the requested and approved ophthalmology services.
3. Study Staff must have full approval from the IRB prior to scheduling.
4. If the subject does not have a CHOP medical record number (MRN), add the subject via OnCore, and request a MRN as soon as the subject is enrolled in the study.
5. Contact Samantha Leuters, Ophthalmology Research Scheduler, via email with a request to schedule. The email must include all requested services, protocol IRB, Current Procedural Terminology code(s) from budget plan, research subject information (name and MRN), and a contact person for scheduling purposes. The Ophthalmology Staff will enter the visit into the EPIC visit schedule.
   1. If an eye exam is included in the requested services, requests must be received by the Ophthalmology Research Staff several weeks (or as required by study protocol) in advance of the date of services to ensure attending physician's availability.
   2. Other tests and procedures will be scheduled in a timely manner and as flexibly as possible within current space and patient flow constraints.
6. Study Staff must enter the visit into OnCore. The visit information line item for Ophthalmology Services should state in the comment field, "Ophthalmology."
7. Study Staff can follow up with Ophthalmology 24 to 48 hours after the visit for results/report.
