Request to Take Computer When Leaving CHOP

Requests to take computer systems must be vetted by our Post-Award Research Administration and Research Portfolio Management. Once approved, a "fair market value" (along with a $50 service fee) is determined for the system in question. The system is "de-CHOPified," all data and applications are removed and the system is returned to a pre-CHOP "consumer state." Please note, any approved data should be backed up prior to turning the system over to have it "de-CHOPified"

To start your request, please click the "Request to Take" button on the left-hand side of this page. In the "description of services needed" field, please fill out that you wish to take your computer when leaving CHOP and include the following information:

  • Make and Model:
  • Serial #:
  • Funding source used to purchase:

Check Request Status

To check the status of your request, go to the CHOP Employee Service Portal (Log in required), under "My Requests", open specific request record and in right column under "Stage" within the request record will be the request status.
