Incident Affects REDCap Data Entered March 27
Situation
Data entered into REDCap between 8:38 a.m. and 9:36 a.m. on Monday, March 27 will appear to be missing. The REDCap team is available to assist with moving data from a backup to production projects.
Background
Users encountered difficulty logging into REDCap and entering data over the weekend through the morning of Monday, March 27. During a fix for this issue, REDCap was unintentionally available for data entry and survey completion, resulting in data being saved to a backup of the REDCap database instead of the main REDCap database.
Research Information Services took a snapshot of the backup database, enabling the REDCap team to access data entered during the affected timeframe in order to assist project teams with moving data back into their projects on the main database.
Action Items
If you or your survey participants may have entered data into REDCap between 8:38 a.m. and 9:36 a.m. on Monday, March 27, 2023, please review your project to see if you are missing data.
- If you are not missing data, no action is required.
- If you are missing a small amount of data that your team can easily re-enter, please do so.
- If you are missing data that is not easily re-entered, please complete this Incident Data Recovery Request so the REDCap team can help move your data.
- If you are an API user and tried to import data between 8:38 a.m. and 9:36 a.m. on Monday, March 27, 2023, please recommit your API import.
Additional Note for Research Projects: As audit trails and timestamps are especially important for research, if your project was affected, we suggest writing a Note to File to explain any resulting discrepancies in the audit trail.
The REDCap team prepared the following sample language for the Note to File:
On 3/27/2023 the CHOP REDCap Team and Research IS reported an issue in which some data entered that morning was saved to a backup database. The CHOP REDCap Team and [Your Study Name] study team worked together to identify this data in the backup and move it to the production REDCap project. Timestamp information in the production project will reflect the REDCap and [Your Study Name] teams conducting data re-entry. If needed for future audits, original timestamp data can be obtained from the REDCap team (redcap@chop.edu).
