
Instructions for Mentor Award Nominations
In submitting nomination letters, please note the following guidelines:
- Nomination packages must contain one to three signed letters from current faculty (not fellows) at the rank of assistant professor or higher
- Mentor Award recipients must have a faculty appointment in one of the CHOP clinical departments: Anesthesiology & Critical Care Medicine, Child and Adolescent Psychiatry, Pathology and Laboratory Medicine, Pediatrics, Radiology, and Surgery
The award is designed to honor:
- Faculty-to-faculty mentoring
- During the past three years of activity (not life-time achievement award)
- Dedication to mentoring regardless of personal benefit or obligation to serve
- Excellence in mentoring various aspects of academic careers such as research, clinical care, administration, teaching, service and strategic career development as appropriate
- Innovation in designing mentoring activities as appropriate
- Mentor's personal academic achievements not a factor for this award
